Right now, we have two levels of care: basic primary care and Functional Medicine Consults. Functional Medicine Consults are 3 month minimum subscriptions meant to solve specific problems on a a consult basis- e.g. perimenopausal symptoms, difficulty with weight loss, getting off of medications. It is possible to graduate to the basic Family Medicine tier when the more time intensive problems are worked through. Functional Medicine Subscription is $200/month.
The basic tier is meant to be a long-term Family Practice doctor-patient relationship. To become a member of the basic tier, there is a $150 sign-on fee per member in the first month to pay for the extensive first visit which is often over an hour, getting medical records, all the “sign on” work that has to occur. If membership is cancelled and restarted, sign on fee applies every time membership is restarted. After that first month:
Adults 18 and older $80/Month
Monthly membership fees for children who sign up with a Parent/Legal guardian $35/mo
Children without a Parent/Legal Guardian. $45/Month

1-4 sessions $35 Each
Four additional sessions $70 each
Any further needed sessions $100
Patients of Diana receive four OMT sessions at $70 each followed by any additional sessions at $100 each.
